The Cobham vision and dental benefit provides coverage for services agreed upon by you and your provider. The plan is a Direct Pay plan administered by TASC.
- There is no waiting periods or pre-approval requirements for eligible procedures.
- There is no maximum fee schedule or limitations for eligible procedures.
- The only procedure exclusions in the plan are dental cosmetic procedures (bleaching) and medical/corrective eye surgery (LASIK). (For coverage information on medical eye surgery please visit the medical benefit section of the website.)
- The plan allows dependent orthodontic care to be covered the same as any other procedure up to a lifetime limit of $1,000.

Cobham provides safety glasses free of charge to those
employees who work in areas that require them.
Providers
The vision and dental plan allows you to use the dentist and
eye care provider of your choice.
Submitting a Claim
Some providers do not submit claims as a matter of policy.
To submit a claim, follow these simple steps:
- Get an itemized receipt.
- Complete the Direct Pay Reimbursement Form
- Send the copy of the recipt and your Direct Pay reimbursement form to the address located on the back of your insurance card.
